HOW TO BOOK:
Go to www.flyfrontier.com and book online. Select 0 adults / 1 child (2,3 children) in the search. Don't redirect from Kayak. Call Frontier if further assistance is needed. Often long holds with no call back option.
WHAT WILL YOU NEED?
Before you call have the following information at hand:

Full name of the minor(s) travelling

DOB and gender of the minor
-
Flight details you want to book
-
Name, 2 phone numbers and physical address of the person dropping the minor off.

Name, 2 phone numbers and physical address of the person picking the minor up. (Addresses should match the ID the guardian is planning to present).

The credit or debit card and billing address.
UM FEE:
$100 each way per child of minors flying together.
It is possible to split the roundtrip fee of $200 into $100 on departure and $100 on return.
Payable on the website. It maybe possible to pay the UM fee at the airport if you call the operator and ask for this arrangement.

OTHER INFO / USEFUL TIPS:
FRONTIER is a "discount airline" offering comparetively low rates but charing for some extras like carry on bags. (It is cheaper to prepay the luggage ahead of time rather then at the airport).
UM ages:
5-14 (unless accompanied by someone 15 or older)
15-17 optional
Guardian minimum age 15
UMs are only allowed on nonstop flights
Red eye is OK if it is only flight of the day
There is an option to add a complimentary "Meet and Assist" service to the reservation. A airline representative will wait for the child at the gate and escort him/her to the lobby. Useful when flying older kids without full chaperone service option.
Guardianship info editable online
UM cannot be charged online - call or pay at airport
They need to speak with the card holder (three-way him in)
Cancelation free 24 hrs. Then $75 fee
ID is not required for passengers below 18 y.o. Only proof of age.
