HOW TO BOOK:

 

 

Call Hawaiian call center at (800) 367-5320 to book with an agent.

Ask the operator if you are eligible for waiving the phone fee since you are not able to make the UM booking online. After the flight is booked, fill out this form and bring it to the airport. If you don't have a printer (or time) this may be done at the airport too.

 

 

WHAT WILL YOU NEED?

 

Before you book have the following information at hand:

 

1. Full name of the minor(s) travelling

2. DOB and gender of the minor(s)

3. Flight details you want to book

4. Name, phone number and physical address of the person dropping the minor off.

5. Name, phone number and physical address of the person picking the minor up.

6. The credit or debit card and billing address.

 

The drop off guardian will be asked to pay the UM fee of $100 each way and fill out the form including Name, phone number and address of the pick up guardian.

 

 

UM FEE:

$100 each way (payable at the airport)

$35 if between the Hawaiian islands

$25 booking by phone

 

GUARDIANS

Only the guardians specified in your booking will be allowed to drop off / pick up the minor upon showing their ID. You may request to change these persons by calling Delta with your confirmation number at least 24 hours prior to departure.

3rd Bag: $100 ($50 on interisland flights)
